There are two ways phasers can miss, 1 is that the computer makes a slight misscalculation and at distances this can mean quite a big miss and two the other ship (has to be a small ship really) can if moving randomly get lucky and avoid a shot however beam phasers are a bit different as they can be re-targeted constantly unless the ship goes out of the phaser arc.
Anyway my point being pulses should miss maybe 1 in 20 or 30 times it would I think be noticable in the game but not a huge anoyance.
Oh and if you want to look at the bullet being fired, if you throw yourself out of the way a split second before its fired its gonna miss you and if the guy is a naff shot and he doesn't aim it right it misses you.
As far as the computer calculating where your going to be they would have to sit and watch you for hours and hours before they managed to work out the random patern that forms your evasive manouvers at that time and if the patern is changing based on another random (I know nothings truely random but it can be cose) number which they dont have the algorithem for there is no way the computer can work out where your going to be perfectly every time which is why pulses should miss as they have no guidence
So pulses should have a hit chance of about 0.95-0.98 and normal beam phasers should have a hit chance of 1
